Systematic Evaluation of Suspected Dementia for Primary Care

This section includes downloadable, writeable series of checklists for use in performing and recording the components of a comprehensive primary care dementia evaluation

(1) an overview of the system

(2) a medication review form

(3) a cognitive evaluation history for a caregiver to complete

(4) two versions of a cognitive examination (provider should choose either the Montreal Cognitive Assessment Basic or the St Louis University Mental Status Examination)

(5) a neurological examination checklist

(6) the PHQ-9 depression screen

(7) a summary form that integrates findings from all other forms into a diagnostic profile of the patient.

In addition to these materials, a diagnosis must take into account the overall clinical picture, including other relevant elements from the past history, current presentation, or laboratory evaluation.
